В статье рассматриваются современные представления о патогенезе МАЖБП и связи этой патологии с нарушениями углеводного обмена, в частности, роль инсулинорезистентности, дисрегуляции аутофагии и другие факторы, способствующие развитию заболевания. Кроме того, анализируются современные терапевтические подходы к лечению МАЖБП, в том числе использование урсодезоксихолевой кислоты (Урсосан) как модулятора процесса аутофагии, а также ряда сахароснижающих препаратов, таких как агонисты рецепторов глюкагоноподобного пептида-1, ингибиторы натрий-глюкозного котранспортера 2-го типа и тиазолидиндионы. Дополнительно описаны патогенетическая роль фактора роста фибробластов-21 в развитии МАЖБП и перспективы клинического применения препаратов на его основе. Таким образом, настоящий обзор отражает актуальные данные о патогенезе и лечении МАЖБП, подчеркивая необходимость комплексного подхода к управлению этим заболеванием в контексте тесной связи с нарушениями углеводного обмена.</p></abstract><trans-abstract xml:lang="en"><p>Metabolic-associated fatty liver disease (MAFLD) is a chronic, non-infectious condition associated with metabolic disorders, including type 2 diabetes mellitus. This review describes current understanding of the MAFLD pathogenesis and its relationship to impaired carbohydrate metabolism, particularly the role of insulin resistance, dysregulated autophagy, and other factors contributing to disease development. Furthermore, it analyzes existing therapeutic approaches to MAFLD management, including the use of ursodeoxycholic acid as an autophagy modulator, as well as various glucose-lowering medications, such as glucagon-like peptide-1 receptor agonists, sodium-glucose cotransporter 2 inhibitors, and thiazolidinediones. Additionally, the pathogenetic role of fibroblast growth factor-21 in MAFLD development and the potential clinical application of FGF-21-based therapies are described.
