Результаты 24-летнего наблюдения за пациентами, завершившими участие в 20-летнем исследовании UKPDS, продемонстрировали значительное преимущество раннего назначения этого лекарственного средства в предотвращении осложнений диабета. В наблюдение были включены 1489 пациентов со средним возрастом 70,9 лет. По истечении 42 лет наблюдения после рандомизации в UKPDS в группе метформина сохранялось численное преимущество в профилактике исходов СД2 по сравнению с группой препаратов сульфонилмочевины / инсулина: любого исхода, связанного с СД (18 против 10%), смерти, связанной с СД (25 против 16%), смерти от любых причин (20 против 10%), инфаркта миокарда (31 против 17%). Эти результаты статистически значимо отличались от группы диетотерапии (p &lt;0,05). Таким образом, у метформина обнаружено первичное профилактическим действие на исходы СД2, которое сохраняется на протяжении практически всей жизни пациента с этим заболеванием.</p></abstract><trans-abstract xml:lang="en"><p>Metformin, long considered a mainstay of treatment for type 2 diabetes mellitus (T2DM), has recently received new evidence of its effectiveness. Results from a 24-year follow-up of patients who completed the 20-year UKPDS study have demonstrated a significant benefit of early metformin administration in preventing diabetes complications. The extension study included 1489 patients with a mean age of 70.9 years. After 42 years of follow-up after randomization in the UKPDS, the metformin group retained a numerical advantage in preventing T2DM outcomes compared with the sulfonylurea/insulin group: any outcome related to DM (18% vs. 10%), death related to DM (25% vs. 16%), death from any cause (20% vs. 10%), myocardial infarction (31% vs. 17%). These results were significantly different from the diet therapy group (p &lt;0.05).
