Широкая распространенность рецепторов к ГПП-1 в органах и тканях организма человека обеспечивает множество значимых плейотропных эффектов арГПП-1. Результаты серии крупных рандомизированных клинических исследований продемонстрировали высокую эффективность, хорошую переносимость и безопасность этого класса лекарственных средств в управлении кардиометаболическим здоровьем, что определило их преферентное положение в клинических рекомендациях по лечению сахарного диабета 2 типа (СД2). Особое место в линейке препаратов класса арГПП-1 занимает семаглутид в связи с его особыми дополнительными эффектами по снижению массы тела и влиянию на крупные сердечно-сосудистые исходы независимо от наличия СД2, что позволило расширить показания к их применению для лечения ожирения и снижения ССЗ и общей смертности. В статье представлен литературный обзор актуальных данных о механизмах действия и метаболических эффектах арГПП-1.</p></abstract><trans-abstract xml:lang="en"><p>Glucagon-like peptide-1 receptor agonists (GLP-1) represent an innovative class of hypoglycemic drugs, the clinical effects of which have gone far beyond glycemic control. The wide prevalence of GLP-1 receptors in organs and tissues of the human body provides many clinically significant pleiotropic effects of GLP-1. The results of a series of large randomized clinical trials demonstrated the high efficacy, good tolerability and safety of GLP-1 agonists in the management of cardiometabolic health, which determined their preferential position in clinical guidelines. A special place in the line of drugs of the GLP-1 receptor agonist class is occupied by liraglutide and semaglutide, due to their special additional effects on weight loss and effect on major cardiovascular outcomes, which allowed to expand the indications for their use in the treatment of obesity and reduction of CVD and overall mortality.
